Provides care coordination services to at-risk & postpartum women in Wicomico County. Services include: home visiting, prenatal education, parenting support, childbirth education, breastfeeding education, access to safe sleep classes & car seat checks, linkages to other community resources.

Thrive By Three (TBT)
Provides services to at-risk pregnant and postpartum women, and families with children up to 3 years old. Thrive by Three increases access to care via maternal and infant care coordination. Services include short-term coordination, home visiting, parental support, education and linkages to community resources and services.
