Who We Are…

Asthma & Lead Case Management is a team of Community Health Workers and Nurse Environmental Case Managers who are trained in assessing and managing Asthma & Lead Poisoning in children. Our team is also trained in identifying and/or assessing asthma triggers, asthma risk factors, lead hazards, lead risk reduction strategies, and other environmental issues that may pose a risk to you and your family.

What We Do…

Our team provides short-term case management, education, and free cleaning supplies to qualifying families who have a child with Asthma or lead poisoning. We meet with families in the comfort of their homes once a month for for a total of 3-6 months.

Asthma Services

  • Provide education on asthma basics, early warning signs & symptoms, and asthma related health risks
  • Help you and your family to identify and reduce/eliminate asthma triggers within the child’s environment
  • Be a bridge between you and your child’s care provider(s) to ensure that your child has an up-to-date Asthma Action Plan
  • Review medications and discuss medication administration techniques
  • Help you and your family to identify and reduce hazards within the home that may be a danger to you and your family (ex: carbon monoxide, mold, lead, poisons, broken railings, etc.)
  • Educate about effective and safe cleaning methods to reduce/eliminate asthma triggers
  • Provide referrals to other community resources
  • Provide FREE cleaning supplies to participating families including mops, buckets, paper towels, cleaning solutions, allergen mattress and pillow covers, pest management supplies, HEPA vacuums, and more!

Lead Services

  • Assist you and your family to identify and reduce/eliminate lead hazards within your child’s environment
  • Provide education about lead poisoning in children, and how to reduce lead levels
  • Discuss proper methods for cleaning and removing lead paint chips, dust, and other remnants
  • Referral to extensive lead abatement (removal) services performed at ZERO COST for qualifying families
  • Follow up with families to ensure that blood lead levels are retested as necessary
  • Provide referrals to other community resources
  • Provide FREE cleaning supplies to participating families including mops, buckets, paper towels, cleaning solutions, pest management supplies, HEPA vacuums, and more!

Who Qualifies?

Families with children who:

  • Are under the age of 19
  • Are currently enrolled or eligible to enroll in Medicaid based on income
  • Spend at least 10 hours per week in your home

AND

  • Have moderate/severe Asthma and/or
  • Have a blood lead level of 5 or above
